1. Make Your Payment

FULL PAYMENT IS DUE on or before Monday June 6, 2007. We use Paypal to securely accept payment via credit card, debit card or electronic check.

Fill in your information and make your payment..

IMPORTANT! : After the payment goes through you will be re-directed to a link for the online
registration form. This form must be completed and submitted immediately following payment.

2. Complete The Online Registration Form

After you make your payment in full you will need to complete the Online Registration Form.
Please fill out the form completely including all medical release information.

Once you have reviewed the form to make sure it is completely filled out, you may click the submit button

3. Download The Information Packet

Once the Registration Form is submitted, you will be able to download our outreach information.
You will want to download and print this packet as it contains very important information about what you will
need to bring along with you on the outreach.
